Data Subject Request Form

This form may be used to submit a request to exercise your rights under the privacy or data protection law that applies to the processing of your personal information. This form should be completed by either the Data Subject (person with relationship with Paylocity) or an Authorized Agent of the Data Subject. 

If you are a current employee of Paylocity, please contact your HR Business Partner to initiate your request to exercise your privacy rights. 

If you are a current or former employee of one of our business clients, please contact that client to initiate your request to exercise your rights. Please also see our Notice of Privacy Practices for Client Employees.
